Knowledgeable Advocates
When you or someone you love is critically ill, the task of understanding what the diagnosis is and what the options are for recovering health can be overwhelming and stressful. It is important for any patient to reduce stress and focus on healing. ConnectCare3 has a staff of professional advocates, nurses and researchers ready to assist you with their extensive knowledge and expertise. We have served patients with a wide range of diagnoses including organ transplants, cancers, neurological conditions, and many more. As we serve patients, our research staff is continually updating and expanding our library with extensive information about multiple types of illness, physicians and facilities.
We have the information and experience you need to help you move toward healing. Don’t waste precious time trying to figure it out on your own. ConnectCare3 exists to fill the information gap so you and your family can focus on healing.
Staff
Each member of the ConnectCare3 staff brings specific, valuable experience to the patient care team. The patient care team is broken into three categories:
- Patient Advocates: Typically the patient’s first point of contact, Patient Advocates assess each individual’s needs and preferences regarding their diagnosis. Patient Advocates also handle non-medical related needs such as assisting with insurance issues and identifying social services that are relevant to the patient.
- Nurse Navigators: Our Nurse Navigators are all registered nurses who have extensive experience in the health care system. Every patient is assigned to a Nurse Navigator who will assist with diagnosis education, identifying treatment options, obtaining a second opinion and appointment summaries (upon request).
- Research Assistants: Our dedicated research staff tracks cutting edge treatment developments, facilities recognized for excellence and physicians with specialized expertise.
Because ConnectCare3 is independently owned and operated, we put our patient’s best interest before all else. Our staff’s only goal is to assist patients with finding the best care available for their diagnosis.

Library
In order to serve each of our patients more efficiently, ConnectCare3 has created a library through extensive research. The information in the library is populated by our dedicated research staff and includes two different categories: health resources and health information.
The Health Resource library includes:
- Expert Physicians and their area of expertise
- Centers of Excellence
- Home Health Care Companies
- Hospice Facilities
- Substance Abuse Treatment Facilities
- Assistance Organizations: Transportation, Meals, Cleaning, etc.
The Health Information library includes:
- Disease specific educational materials that explain symptoms, treatment options, etc.
- Disease specific tools for patients that assist them with following their chosen course of treatment
- Research papers from Centers of Excellence about specific illnesses
- Articles from multiple sources outlining the most recent advances in treating specific illnesses
