ConnectCare3 is a personal, confidential patient advocacy and nurse navigation service designed to assist patients and families dealing with serious illness. Our staff can help you:

Because ConnectCare3 is independently owned and operated, we put our patient’s best interest before all else. Unlike case managers employed by insurance companies or advocates employed by medical facilities, ConnectCare3 only answers to our patients. We are not looking to cut costs or liability for our employer. Our staff’s only goal is to assist patients with finding the best care available for their diagnosis.

ConnectCare3 also provides face-to-face interaction with our Nurse Navigators in addition to phone and email support. The Nurse Navigator will attend physician and hospital appointments at the request of the patient or family. After each appointment, the patient is provided with a summary of the physician’s comments and recommendations in easy to understand terms. Nurse Navigators are a source of information during appointments while providing a sense of calm, control and understanding during a highly stressful and confusing time.
