ConnectCare3 helps you create and maintain an organized format for all of your medical records, keeping you informed and prepared for every medical situation or health eventuality. Creating a Personal Health Record is easy and immediate.
Individual Plan Members - After you enter your doctors, emergency contacts, medications, medical conditions and any other pertinent information about your medical history into our system, we do the rest. We will do the work of gathering and processing all of your records, paper or electronic, X-Rays, MRI’s or Dental Records– everything relating to your health history – and then transform them into digital images. This is done by experienced professionals who understand the sensitivity of this information and are contractually bound to confidentiality. We then index the records by date, provider, and record type, or by a customized theme, so you, your doctor, your pharmacist, your caretaker - anyone you choose - can find the relevant medical information they need, quickly and easily.
DIGITAL HEALTH PROFILEThe Digital Health Profile is a Personal Health Record (PHR) that includes information about you, your allergies, family history, medical conditions, emergency contacts, pharmacies, healthcare providers, and many other facets of your health.
