Personalized care coordination means that our Nurse Navigators take into account your personal preferences when compiling treatment options and are available for face-to-face interaction upon request. In addition, ConnectCare3 will identify social services that are available to help with the needs of daily living while patient recuperate. Personalized care coordination from ConnectCare3 includes:
- Identifying each individual’s goals for regaining their health and assisting individuals in meeting their health goals
- Supplying a Care Journal to patients with complex conditions to assist them as they keep detailed records of their physicians, medications and medical tests in case of an emergency.
- Researching each individual’s insurance plan to ensure that all treatment options and the costs associated are represented accurately
Personal Preferences:
ConnectCare3 recognizes that each individual has different preferences when it comes to their health care. Preferences might be determined by your lifestyle, family commitments or job requirements. Our staff has been trained to assess your health care preferences when you enter our intake process. This allows our Nurse Navigators to ensure that the options presented are in line with not only your medical needs, but also your preference for treatment. Some of the questions our staff may ask include:
- How far are you willing to travel for care?
- What type of treatment facility do you prefer?
- Do you want to consider holistic options?
- Are you willing to join a clinical trial?
- Do you prefer to attend appointments alone or with a nurse navigator?
- Do you want questions prepared in advance to help you communicate more effectively with the physician?
Face to Face:
The age of technology has brought many ways to communicate including emails, texting, skype, facebook, twitter and more. But when you or one you love is facing a health crisis, it can be reassuring to have a caring, compassionate nurse navigator talk to you in person. Every patient using ConnectCare3’s services has the option to request that their Nurse Navigator accompany them on appointments with physicians and treatment facilities. This service is an invaluable part of what makes ConnectCare3 so special. The nurse navigator provides a source of information during the appointments. After each appointment, the nurse will provide a summary of the physician’s comments and recommendations in easy to understand terms. In addition, nurses can provide the patient with a sense of calm, control and understanding during a highly stressful and confusing time.
Social Services:
Adhering to the saying, “Don’t sweat the small stuff” can be easier said than done. ConnectCare3 recognizes that illness can affect individuals and their families in multiple ways, including many that are not necessarily considered health-related. Everyday needs that once were simple, can seem impossible when you are diagnosed with a life altering illness. Examples include, but are not limited to:
- Cleaning
- Babysitting
- Transportation
- Meals
- Housing during out-of-town treatments
