Navigating the healthcare system can easily become overwhelming for patients diagnosed with serious illnesses or conditions and their families. Enter ConnectCare3, a patient advocacy service located
in Lititz, PA which provides personalized healthcare navigation to those who need it most.
Each patient has access to a Patient Advocate, Nurse Navigators and a Community Service Coordinator. These professionals help patients better understand their diagnosis, explore treatment options, identify centers of excellence, find highly rated physicians and assess the availability of social services.
Ann Linkey, Research and Data Specialist for the company, says the level of patient interaction provided is unique and significantly improves the overall patient experience for clients. “We will meet with patients in their homes and even go with them on doctor visits and to the hospital when needed,” said Linkey. “Our goal is to make sure the right questions are asked so that the patient gets the best results possible. We provide support during the appointment and can also prepare a written summary of what transpired for the patient.”
She also pointed out that efficient data storage and retrieval played a critical role in establishing and growing the service. “We have to be able to store and access all of the information that a patient needs to make an informed decision,” said Linkey.
When searching for the right database system to meet this need, Linkey explored systems in use by other social service agencies in the state and ultimately selected ServicePoint® 5.0. “One strong advantage to ServicePoint is that it is a well-known and widely used system, so I was able to talk with others

The ClientPoint module is used to track patient information. “With ClientPoint in place, we are able to collect and store a large amount of patient data, to customize the assessments and to insure confidentiality,” said Linkey.

Another module, ResourcePoint, is also utilized to store information about the availability of resources such as hospitals, other medical facilities, social services and national organizations offering specialized resources for severe illnesses and conditions. “ResourcePoint allows us to store resources that we want to be able to refer to regularly,” explained Linkey. “And since it is a web-based system, our nurses can access it when they are out meeting with clients.”
The company went live with the system in January 2011 and has enjoyed a smooth transition from a very basic database to the new system. “A big factor in the ease of use for us was the fact that Bowman Systems sent a trainer to us,” said Linkey. “Having someone here in person doing the training makes a huge difference, and our trainer, Levin Corbin, knows ServicePoint inside and out.”
Linkey also plans to use ServicePoint to track trends such as treatment costs and preferred treatments using the system’s reporting tool, ART. “ServicePoint has the capacity to grow with us,” added Linkey. “This is important because it means we won’t have to upgrade to a new system as we continue to evolve. This is a system that can be easily adapted to meet changing needs.”
