Michelle Gilmore - Patient Advocate
When someone contacts ConnectCare3 with a life-threatening illness, I feel great empathy for their situation. I am their first point of contact and their first impression. During our conversation, I strive to instill confidence that we will exhaust all possible resources to help them make informed decisions. As I hand their file over to a Nurse Navigator, I hope I have laid the foundation of the compassionate care they will receive throughout their experience. My role as a Patient Advocate of ConnectCare3 has made me appreciate the gift of health more than ever.
Stephanie Mesaric - Nurse Navigator
As a Nurse Navigator I am unable to take away the disease, but I am able to empower my patients with information to help them make the most informed decisions for their particular issue. I am proud to be a Nurse Navigator with ConnectCare3 because of the unique ability to provide patients access to a wide community of services, not only these affiliated with one particular institution. In today's fast-paced world, we all need to know that we matter. Working as a Nurse Navigator provides me with the opportunity to let my patients know that their health and recovery is the only thing we care about.
