- Nurse Navigator – Your nurse navigator meets directly with you and does research to help you be fully informed and supported. She will help you understand test results and go along on doctor visits if needed.
- Social Services – By meeting with you personally and understanding your needs, the ConnectCare3team can connect you to useful social services in your community that will help you in this trying time.
- Medical Panel – Should you case involve complex medical questions, you and your Nurse Navigator may send the entire case file to our accomplished ConnectCare3 medical panel of first rate doctors for review.
