What exactly is CC3?
CC3 is a service designed to assist individuals with serious healthcare issues. When you determine that you have a serious need we are there to help you understand the diagnosis and treatment options. Our medical professionals and community service experts are available to guide you and your family through the complex healthcare process and achieve the best possible outcomes.

CC3 is not a healthcare provider, but rather a service that enhances your employer’s or your own healthcare plan.


How does all this work?
Contact CC3 at 877.223.2350 on business days from 8:00 until 5:00. Our Patient Advocate will evaluate your situation and assure your privacy throughout the conversation. Our Patient Advocate will then consult with both our Nurse Navigator and our Community Service Coordinator to begin to open doors for you, research the best options, and assist you in making the best decision for your situation.

Your Patient Advocate will:

  • Review your current medical situation and treatment history
  • Review your health insurance coverage
  • Work to reduce any anxiety and fear
  • Begin to investigate your options
  • Engage the Nurse Navigator and Community Service Coordinator

Your Nurse Navigator will:

  • Research and provide you information concerning appropriate centers of treatment excellence and the most highly rated physicians
  • Inform you of the most advanced treatment options
  • Facilitate timely access to treatment
  • Prepare you for your medical appointments to ensure that your questions and concerns are addressed
  • When appropriate and requested accompany patients to medical appointments
  • Help patients understand the answers to their medical questions and diagnostic results
  • Assist patients in understanding and following their treatment plans

Your Community Service Coordinator will:

  • Research and provide the patient and their family information on the availability of the community services that will ensure the best outcomes
  • Assist the patient and family in accessing desired community services
  • When appropriate and requested accompany patients to community service appointments

 

How do I know if I qualify for CC3?
Simply call us at 877.223.2350. Your Patient Advocate will assess your situation and answer your questions. This service is for patients who believe they have a serious illness and want the help of a professional team. If you qualify there is no additional charge for using the services of CC3.


What information do I need to have when I call ConnectCare3?
Be prepared to discuss your diagnosis and any recommended treatment plan. Have the names and contact information for any healthcare providers with whom you are working. Be prepared to verify your health insurance coverage. Allow twenty minutes of time to allow for a complete assessment. CC3 is available on business days from 8:00 a.m. until 5:00 p.m.


What about confidentiality?
The conversations between you and ConnectCare3 are treated as highly confidential and handled in the same manner as those with your most trusted physician. We abide by all HIPAA regulations.


Who is on the staff of CC3?
The staff at ConnectCare3 consists of the following:

Registered Nurses whose sole responsibilities are to help patients/family members navigate the complex healthcare system.

A Patient Advocate who is responsible for completing the initial assessment when someone calls for service as well as verifying insurance information.

A Community Service Coordinator who is responsible for researching all the community service agencies in the communities where we are serving patients.

A Medical Panel comprised of a multi-disciplinary team of physicians who work side-by-side with the nursing staff if/when another review of the case is necessary.

 

If CC3 informs me and my family of an advanced treatment center, am I obligated to go there?
Absolutely not. YOU make the decision about your health care. Whatever the decision is, CC3 is prepared to assist you and your family. Our role is to inform you of your treatment options so YOU can make the most informed decision possible.
